In September we sent out a copy of your student’s data to verify that the information is accurate and up to date.  As part of a data review, we are sending an updated paper copy (on purple paper) of your student’s Data Verification Form home with each student.  This form verifies your child’s personal information as well as your contact and emergency contact preferences.  This information will be used as part of our family portal initiative, which will be implemented in the future.

Please review the information on the purple paper copy of the Data Verification Form, make appropriate changes, sign the bottom of the form, and return it to the school by Thursday, March 28th, 2024.  The form should be signed by the family member or caregiver with lawful access to the student and will act as an attestation of the accuracy of the information.  Even though families submitted a Data Verification Form at the start of this school year, we do require this update and it is mandatory for all students.

An instruction sheet (on yellow paper) will accompany the purple Data Verification Form to help answer any questions you may have.  If, after reading the instructions, you have further questions, please contact KCI’s main office.
